Getting Started

To request an initial consultation, please call 303.499.0331 and leave a message with your name and telephone number. I will return your call and initiate a brief phone consultation to determine if proceeding with an initial visit makes sense. Initial visits are consultative in nature, and the consultation period may occur over several visits. This consists of collection of comprehensive physical and mental health information, preliminary assessment of the nature of the problem, and recommendation for further testing or treatment. In some circumstances, I may recommend a different treatment provider and/or treatment setting as more suitable for your needs. Once we have mutually agreed to work together, you will become an active patient.

Payment

Payment for services is due in full at the time of service. Accepted methods of payment include cash, check, or credit card.

Insurance

I am not an in-network provider for any managed care organizations or insurance companies. Per Colorado state regulation, I cannot provide and bill for services to Medicaid-insured clients even if they wish to self pay. Medicare-insured clients who wish to self pay can be seen. All clients will be provided with the necessary documentation should they choose to seek reimbursement from insurance company for an out-of-network provider or from an HSA/FSA.  

YOU MAY WANT TO ASK YOUR INSURANCE COMPANY THE FOLLOWING QUESTIONS REGARDING OUT OF NETWORK REIMBURSEMENT: 

  • Do I have mental health benefits?

  • Does my plan offer out-of-network benefits that will reimburse me if I am treated by an out-of-network provider? If so, what percentage of the cost will I be reimbursed? Is my reimbursement based on a maximum allowable charge determined by the insurance company?

  • How long will it take for me to receive reimbursement?

  • Is my benefit limited to “parity” diagnoses (certain psychiatric conditions which are covered the same as medical diagnoses)?

  • What is my deductible and has it been met?
